Client Care Coordinator, Home Care Services

Coordinates and supervises the delivery of the agency’s Home Care services.  Markets services to community groups and organizations. Full Job Description.

• Coordinates the delivery of the agency’s Home Care (Private Duty) services to the community as well as to existing clients of the Agency.
• Organizes and directs services to ensure compliance with all state standards for Home and Community Support Services licensure
• Employs qualified personnel to provide services and ensures adequate staff education and evaluations.
• Ensures documentation of all client records is accurate and timely in accordance with agency policies and state licensing standards.
• Ensures a reassessment of a client’s need is performed according to state licensing standards.
• Ensures client’s service plan is executed as written.
• Ensures services are initiated within established time frames.
• Works closely with the Administrator of Home Care Services on department issues or problems.
• Responsible for keeping Department administration informed on all events, which may occur.
