Geriatric Care Management and Consultation assists family caregivers with issues they are facing with an elderly loved one. Services may include an in-home assessment, customized care plan, coordination of services, and monitoring to see that any emerging needs are addressed.
In Home Care provides seniors with private duty services in the home, such as personal care assistance, companionship, home management and transportation. Home Care helps seniors maintain their dignity and independence in the comfort and security of their own homes.
Home Health Care provides seniors with Medicare-certified nursing, physical, occupational and speech therapies, a comprehensive dementia care program, disease management programs, home health aides, medical social work and home medical equipment, as prescribed by a doctor.
Lifeline Medical Alert Service provides 24-hour monitoring for safety or medical emergencies through a waterproof, wearable personal help button.
The Alzheimer's Day Center provides seniors who have Alzheimer's disease or other forms of memory loss with caring supervision. The center provides health services, opportunities for socializing, and a variety of specialized activities in a secure and well-planned environment.
Case Management helps seniors who want to remain living in their own home, by arranging the appropriate community services.
Housing & Energy Management helps low income seniors remain living in their own homes by providing energy efficiency measures and education, and assistance with utility bills.
Telephone Reassurance provides seniors with safety-check phone calls, made Monday through Friday.
Volunteer Services enhance the quality of life and safety of seniors who have limited social contacts and resources. Programs include Telephone Reassurance, Adult Day Center and Give a Gift. Volunteers are also needed for Special Events.
