Finding vital services for the elderly often involves complicated applications and confusing eligibility requirements. Dealing with serious health and financial issues while trying to locate services can be overwhelming and frustrating.
Case Management services are provided to elders at the greatest risk of losing independence and being placed in an institution unnecessarily.
Clients are typically female, homebound, and living alone on a very low income. Most have multiple and chronic health conditions limiting their ability to care for themselves. Without outside assistance, basic needs for food, utilities, safe housing, and adequate health care would go unmet.
Sheltering Arms case managers have expert knowledge of aging and its problems, and of elder care community resources. Case managers use their expertise to:
- conduct a comprehensive evaluation of each client's situation
- identify and prioritize problems
- develop a care plan that identifies community services which can address needs
- arrange, coordinate, and monitor the needed services to ensure issues are addressed
Eligibility Requirements
- Age 60 or older
- Resident of Harris County
- Living in house or apartment (not a facility or nursing home)
- Experiencing difficulty meeting basic needs due to:
- Financial constraints
- Limitations due to physical or cognitive impairments
- Low social support
Gatekeeper Program
The Gatekeeper Program is a collaborative effort between the Sheltering Arms Senior Services, the United Way, and several other companies and agencies. Public service and utility workers who encounter the elderly in the course of their day-to-day work are trained to recognize potential problems and refer frail and isolated elders for help. Individuals needing case management are served by Sheltering Arms Senior Services.
